Resumen


Desarrollos combinados en farmacogenética, farmacogenómica y áreas afines hacen plausible un nuevo enfoque de la asistencia sanitaria, caracterizado por la posibilidad de combinar diagnósticos moleculares, perfiles genéticos y datos clínicos para mejorar la eficacia y seguridad de los tratamientos farmacológicos. Se habla de “medicina individualizada” porque las prescripciones y seguimientos individualizados en función del genotipo pueden contribuir a reducir los riesgos de toxicidad y reacciones adversas severas asociados a las “medicaciones de talla única” convencionales. El manejo de consideraciones étnicas y el recurso al genotipado en los ensayos clínicos será cada vez más frecuente porque la variabilidad en los polimorfismos genéticos de individuos y poblaciones se traduce en diferencias farmacocinéticas y farmacodinámicas. Sin embargo, la estratificación o clasificación de sujetos de investigación en función de su genotipo es una cuestión debatida y compleja, que puede comprometer la validación de los tratamientos asociados a pruebas genéticas y dificultar la apreciación de validez analítica y utilidad clínica en los protocolos que llegan a los comités de ética de la investigación clínica (CEIC).
